The Centre for Seniors’ Medical Psychiatry is an outpatient program integrating Geriatric Medicine and Geriatric Psychiatry anchored in primary care, where the primary care practitioner remains the most responsible provider. 
  Under the supervision of geriatric specialists, RN Care Managers or allied health professionals will work with the patient and the primary care provider to manage both the physical and mental health needs with a collaborative care intervention for up to 16 weeks.
Who We Help
  Seniors who live in the Mississauga Halton region who experience a physical health condition affecting function 
and depressed mood and/or anxiety can be referred.  
  A physician or nurse practitioner referral is required.
