Social Work

Master's level social workers provide service in 20 inpatient and outpatient programs throughout the hospital community.

Members of the multidisciplinary team, provide psycho-social assessment, treatment and access to community services. As well, social workers facilitate discharge plans.

Services:

  • Crisis Intervention Team
  • Consultation service to ER physicians, providing psychosocial assessment, referral, brief treatment and follow-up to patients in crisis
  • The Addictions and Concurrent Disorders Centre
  • Structured day treatment program for outpatients
  • Other treatments include: information session, relapse prevention, family intervention series and community outreach
  • Quick Response Program
  • Help to avoid unnecessary hospitalization for those patients who arrive in ER but are able to return home with community-based supports and services
  • Provide a mechanism for early identification of admitted patients needing discharge planning assistance
  • Work in conjunction with the crisis intervention team

Crisis Intervention Team

The Crisis Intervention Team consists of highly skilled social workers specializing in crisis intervention. The team provides consultations to the Emergency Room physician for patients registered in the Emergency Department and experiencing mental health and/or psychosocial crises. In addition to assisting in the assessment process, the CIT provides crisis intervention counseling and linkage to community resources and hospital services as needed.

Telephone crisis coverage includes consultation and community resource information to family physicians and crisis counseling to persons and/or family members who may call the hospital in crisis. On weekday evenings and weekends, the crisis team also provides crisis coverage to the inpatient units.

A crisis worker is on site in the ER, Monday-Friday, 9:00 a.m. to 11:00 p.m. and Saturday, Sunday and Statutory holidays, 11:00 a.m. to 11:00 p.m.

The Addictions and Concurrent Disorders Centre

Mandate

The program offers treatment for people at moderate to severe risk due to addiction to alcohol, prescription medications, cocaine, cannabis, and various street drugs. We accept people who are participating in methadone treatment and being monitored for this elsewhere. We support and treat people with concurrent mental health or chronic medical issues that affect recovery.

  • Collaborative treatment with other CVH outpatient and inpatient psychiatric, ER and medical services is offered where appropriate
  • Continuing care for suitable clients who have completed treatment elsewhere
  • Collaboration with suitable community services.

Expertise

The treatment staff have specialized mental health and addictions training in addition to advanced professional degrees. Staff includes:

  • Physicians with expertise in addictions and mental health
  • Psychiatrists with concurrent disorder assessment and treatment expertise
  • Social Workers
  • Nurses
  • Consultation with other hospital medical and mental health professionals (in specialized programs) where required to enhance treatment.

Treatment Features

  • Support people from early recovery to maintenance following the one year formal treatment continuum
  • Provides both harm reduction and abstinence based treatment based on individual suitability
  • Diagnostic and treatment capability for both mental health and addictions On site toxicology screening
  • Capacity to monitor complex medical withdrawal in community or in hospital where appropriate
  • Capability in management of medical conditions impacted by addiction/recovery processes
  • Provide concurrent disorder treatment following best practice guidelines
  • Comprehensive assessment and referral to most suitable alternate service when indicated

Mississauga Hospital

At Mississauga Hospital, Social Workers deliver services to address the psycho-social, emotional and environmental needs of clients and their families to develop, restore, maintain and/or enhance optimal well-being, functioning, coping with health-related factors and transitions. Social Workers practice autonomously and in collaboration with other health care professionals in a wide range of settings with a variety of client populations and can be found in outpatient, acute and long term care areas, with representation in all Health Systems in the hospital.
