THP has partnered with SeamlessMD to provide a free mobile digital education tool that assists you with education, reminders, and check-in surveys before, during, and after your surgery. It is currently available for patients following certain surgeries and conditions.
What is SeamlessMD?
Seamless is a free, interactive, step-by-step online patient education tool. You can easily access it as a mobile app on your smartphone, tablet and/or online as a website on your computer.
The SeamlessMD education tool has been customized by THP to provide surgical patients like you with the information you need to prepare for and recover from your surgery at Trillium Health Partners (THP).
On SeamlessMD, you get:
- Reminders about how to prepare for surgery
- Tips for recovery
- Daily to-do lists
- Daily health check surveys during your recovery, to report your symptoms and get tailored feedback on what to do next
- A library of useful resources to answer your questions before and after surgery.
Accessing SeamlessMD
If your healthcare team has advised that SeamlessMD is available as part of your care journey, they will provide an overview of the program, sign-up instructions, and any additional support materials needed to get started.
Ways to Access SeamlessMD
Mobile App
Access SeamlessMD using the SeamlessMD mobile app, available for Apple and Android devices.
Online
Access thp.seamless.md SeamlessMD through a secure web browser on your computer, tablet, or mobile device.
MyChart
Some patients may also be able to access SeamlessMD through MyChart. If this option is available to you, your healthcare team will provide additional information.
