The Paediatric Supportive Care Program at Trillium Health Partners provides care to children with a serious illness and their families.
The Paediatric Supportive Care Program is a team of health care providers focused on enhancing quality of life. We listen to the needs of our patients and families and work together to meet them. We aim to help with the symptoms, distress, and stress of serious illness. Our involvement can take place at the same time as other treatments; it does not depend up on the course or stage of your child’s illness.
We work with your existing health care providers to enhance the care your child is already receiving to make sure it reflects the values and preferences of your child and your family, and to maximize quality of life and comfort for your child.
Should end-of-life care be needed, we will be there to ensure your child is comfortable, provide your family with support in your preferred setting, and help create meaningful legacies/memories. We will continue to support you through bereavement and help you to connect with supports in the community.
Who we help
We provide support in hospital, in clinics, and at home to any family with a child under 18 years of age who has been diagnosed with a serious illness.
Location
We are located at the Credit Valley Hospital and see children and families across all Trillium Health Partners sites, as well as in families’ homes, across Mississauga and Halton.
Referrals
Referrals can be made by any member of your child’s health care team by calling (905) 813-1100 ext. 816573 or emailing pact@thp.ca.
