Paediatric Diabetes Program

The Paediatric Diabetes Program at Trillium Health Partners provides family centered support and treatment for both individuals up to 18 years of age and their families dealing with a new diagnosis of childhood diabetes or existing Diabetes. The program supports outpatient diabetes management for both Type I and Type II Diabetes.

The interdisciplinary team of physicians, nurses, dietitians and social workers collaborate with patients and their families to provide the following services:

  • assessment and treatment of paediatric diabetes (type I and II)
  • medical interventions
  • resource planning and support
  • nutrition counselling
  • diabetes management and education
  • insulin pump program
  • a focus on mental wellness and coping

A direct referral from a physician (family doctor or paediatrician) is required and can be faxed to the clinic. All appointments require your child’s health card, blood glucose meter, logbook and a list of all medications that your child takes.

If you have a medical emergency, please contact your child’s family doctor or paediatrician, or take your child to the nearest emergency department. If your child is ill with a communicable illness, please call the clinic prior to your child’s appointment
